Child's Name: Enter the child's surname, first name and middle name. Surname and first name are mandatory (must be entered).
Gender: Select the correct gender for the child. This is mandatory.
Date of Birth: Select the down arrows next to each box then select the correct day, month and year to enter them into the boxes. This is mandatory.
Are any of your child's details incorrect?: You may need to change some of the information on this screen - for example if the child’s address or circumstances change or the wrong date of birth was entered. However if, for example, you are making an application using a UID, some of the information cannot be changed. Also, if the application has been submitted it is not possible to change the information. If this is the case and any of the information relating to the child is incorrect, please type the details in the space provided here to inform the Local Authority.
Is your child's home address the same as yours?: The first time you enter details on this page you have the option to select Yes or No. If you answer Yes, your correspondence address (applicant address) will then be entered automatically as the child's address on this page. To remove the address, select No. If you answer No you will then need to enter the child's address below. If you and the child subsequently move house you will need to enter your new correspondence address and the child's address separately.
Address: If you have confirmed that the child lives
at your address, the address will be entered automatically. If the application
has been made with a UID this field will be filled automatically but can be
modified up to the closing date for the admissions group. After the admission
group closing date applicants should contact the Local Authority directly with
changes to the address. This information is mandatory.
Enter the postcode
then select the search button to open the Address Selector which you should use
to enter the child's address. If you have forgotten the postcode select the link
provided - this takes you to the Royal Mail website where you can search for the
postcode.
If the child currently lives outside the area of the Local Authority to which you are applying, but is part of a returning Crown Servant or Service family that can provide official written confirmation that they will be moving into the area before they start their new school, their new address will be used for the admissions assessment.
Depending on the policy of your Local Authority, there may be a question on this screen to be answered if the child will be moving in the near future. If so, and you know that the child will be moving then you should answer “Yes” to the question and enter the moving date in the boxes provided. You may be asked to enter the child’s new address or to provide it separately - please see the on-screen instructions or contact your Local Authority for further advice if necessary.
As Home-School Distance is often used as a criterion to determine admissions, even if the move is within the Local Authority area, the Authority must be notified of any change in the child’s home address.
Service/Crown Servant Personnel button: If you need to record that the child’s family are Service or Crown Service Personnel (to justify why the child will have a different address when attending the school), select the Yes button. You will need to provide proof of the posting to the Local Authority.
Current School: If you are applying with a UID, the
child's current school should be named here. If, however, this is incorrect
because the child has changed school recently, please use the search facility to
enter the correct school. If you are applying without a UID, use the search
facility to enter the school your child is currently attending.
Select the
Search button to open a list of schools. Select the correct
school name to enter it into the box.
This entry can be modified if the
application has been made with a UID up to the closing date for the admissions
group. After the admission group closing date applicants should contact the
Local Authority directly if this changes.
Relationship: Select the down arrow to display a list of parent/carer relationships. Select the correct relationship to enter it into the field. This is mandatory.
Parental Responsibility: If you have legal parental responsibility for the child select Yes. If not, select No.
Statement of Special Educational Needs: If the child has a Statement of Special Educational Needs (SEN), select Yes. Please note - if your Local Authority has decided to enter this information themselves this box will not be available for you to use.
Fostered/Looked After Child question: This question asks if the child for whom you are making an application is in the care of a local authority (this need not be the authority to which you are applying). Select Yes or No as appropriate. Please note - if your Local Authority has decided to enter this information themselves this question will not be displayed. If you answer Yes then you must also enter the name of the Local Authority that looks after the child. Use the Search button provided to open a page that allows you to search for and enter the correct Local Authority name.
